Why "Just Listen to Your Body" Doesn't Work for Everyone: Interoception, Hunger, and Hydration

If hunger and thirst cues feel faint, delayed, or confusing, the problem isn't willpower, it's interoception, and there are concrete workarounds that help.

You've probably heard the advice a hundred times: eat when you're hungry, drink when you're thirsty, rest when you're tired. It sounds simple, almost too obvious to write down. But for a lot of neurodivergent people, especially those with ADHD or autism, that advice quietly assumes something that isn't always true: that the body's internal signals arrive clearly and on time.

They don't always. And when they don't, "just listen to your body" isn't helpful advice, it's a instruction to read a signal that may not be coming through.

What interoception actually is

Interoception is your sense of what's happening inside your own body. It's how you know you're hungry before you're starving, thirsty before your mouth is dry, tired before you're exhausted, or that your heart is racing before you consciously notice you're anxious. It works alongside senses like sight and hearing, but it's easy to overlook because it operates quietly in the background.

Research in both ADHD and autism communities points to interoception often working differently than the "typical" pattern. For some people, signals are muted or delayed, so hunger doesn't register until it turns into irritability, shakiness, or a sudden crash. For others, everyday internal sensations can feel loud or overwhelming, making it hard to tell one signal apart from another, so mild hunger and low-grade anxiety can feel identical. Either pattern makes it genuinely difficult to use "how do I feel" as a reliable guide for when to eat, drink, or rest.

This matters because so much everyday health advice leans on body awareness as the starting point. If that starting point is unreliable, the advice built on top of it stops working, and it's easy to internalize that as a personal failure rather than what it actually is: a mismatch between the strategy and how your body communicates.

How this shows up day to day

A few patterns come up often in coaching conversations:

Skipping meals without meaning to, then hitting a wall of fatigue, irritability, or brain fog in the afternoon, followed by intense hunger that feels sudden even though the body has actually been under fed for hours.

Going long stretches without water, especially during focused or hyperfocused work, and only noticing thirst once it shows up as a headache or difficulty concentrating.

Confusing hunger with anxiety, boredom, or overstimulation, which can lead to either under eating because the cue never gets correctly identified, or eating in a way that doesn't actually address what the body needs.

Feeling fine one moment and completely depleted the next, with very little warning in between.

None of this is a discipline problem. It's a signal problem. And signal problems respond better to structure and cues than to willpower.

Working with the body you actually have

The goal isn't to force interoception to work differently. It's to build external supports that do some of the noticing for you, so you're not relying on a signal that may be unreliable.

A few approaches that tend to help:

  • Eat and drink on a schedule rather than waiting for a feeling. Anchoring meals and water breaks to the clock, or to existing routines like the start of a work block or a transition between tasks, takes the guesswork out of "am I hungry yet."
  • Use visual or physical reminders. A marked water bottle that shows how much you should have finished by a certain time, a phone alarm labeled simply "eat something," or a water bottle kept in your direct line of sight can all do the noticing that your body might not reliably do on its own.
  • Keep low-effort food readily available. When hunger does register, it can arrive suddenly and intensely. Having something simple and ready, rather than needing to plan or prepare in that moment, makes it much easier to respond well.
  • Build meals around consistency rather than variety, at least as a baseline. Many neurodivergent people find that eating similar meals on a predictable rotation reduces the daily decision load, which frees up energy for noticing how you actually feel.
  • Pair new habits with an existing routine. Attaching a water break to something you already do reliably, like checking email or switching rooms, is more sustainable than trying to remember it in isolation.
  • Separate the feeling from the action when you can. Instead of asking "do I feel hungry," try asking "how long has it been since I ate, and does that number make sense." Letting time, rather than sensation, do some of the deciding can be a useful bridge while you build more awareness over time.

None of these require perfect body awareness. They work by shifting some of the responsibility from an internal signal that might be unreliable onto external structure that isn't.

A note on medical care

Lifestyle strategies like these can meaningfully support how you feel day to day, but they're a complement to medical care, not a substitute for it. If you have a diagnosed condition, take medication, or are working with a clinician on ADHD, autism, or a related diagnosis, any changes to that treatment or management plan should go through your physician or care team, not be self-directed based on a blog post. If patterns around eating, drinking, energy, or body awareness are significantly affecting your daily life, that's worth raising directly with a doctor or a specialist who knows your full picture. Lifestyle changes work best alongside good medical care, not in place of it.

Interoception differences aren't a flaw to fix, they're a different way of receiving information from your body. The goal isn't to force it into a typical pattern. It's to build a routine around the body you actually have, so that the basics like eating, drinking, and resting happen reliably, whether or not the signal shows up on cue.
